To make life a little easier for our customers, we’re offering the following forms in PDF format. These files may be printed and filled out at home at your convenience, then returned to the CGUD office with the proper form of identification. Photo identification (2 forms of ID required) and proof of ownership (such as closing paper, deed or tax receipt) is required for new service applications. If you are renting, you will need to provide the rental agreement or rent receipt showing landlords name.

Identification showing date of birth is required for senior citizens discount applications. (Only one discount per customer).

Note: All forms must be signed in person at the CGUD office to verify the identification of the applicant.
